Geriatric Case Navigators/Care Managers
Geriatric case navigators/care managers provide a wide array of services for older adults. Most conduct a home assessment and then create a recommended care plan based on the interview. The goal of a case navigator or care manager is to set up services that allow for a safe environment for the senior living at home. Once living at home is no longer an option, they can coordinate placement into other settings providing a higher level of care.
